Download a sponsorship form

Use this form to collect donations for CHAS. Sponsors can increase the value of their donation by 25% at no extra cost by ticking the Gift Aid box, if it's applicable to them. Download your sponsorship form here.

Set up a JustGiving page

If your fundraising event involves collecting donations online, we recommend setting up a JustGiving page.

All money raised on JustGiving automatically is sent to CHAS, so you don’t need to worry about how to get it over to us.

To get the most out of your JustGiving page, our top tips to get started would be: 

  • Include a picture of yourself so people are confident they are on the correct page, this also adds a human element. 

  • Tell people why you’re taking on the challenge – this could include why you’ve chosen to support CHAS and why this challenge in particular. 

  • JustGiving studies have shown that by making the first donation, you’ll likely raise 40% more - there’s no pressure to do this though. 

  •  Share it far and wide – across any social media platforms you use, make sure to email friends and family who aren’t on JustGiving too! 

  • If you’re not on social media, why not make up posters with a QR code to circulate round your workplace?
